Job Description: Medical Case Manager
Position Overview:
The Medical Case Manager plays a crucial role in coordinating and advocating for individuals receiving medical care and support services. This position requires a compassionate and dedicated professional who can effectively assess clients' needs, develop care plans, and ensure the delivery of comprehensive medical and social services. The Medical Case Manager collaborates with interdisciplinary teams, medical professionals, and community resources to provide the highest level of care and support to clients within the social work and human services sector.
Responsibilities:
1. Assess clients' medical and psychosocial needs through comprehensive interviews, evaluations, and assessments.
2. Develop and implement individualized care plans, focusing on the integration of medical and social services to ensure optimal client outcomes.
3. Coordinate and facilitate access to healthcare services, including medical appointments, treatments, diagnostic tests, and medications.
4. Advocate for clients' rights and entitlements, addressing barriers to healthcare, and promoting effective communication between clients and healthcare providers.
5. Collaborate with interdisciplinary teams, including physicians, nurses, therapists, and social workers, to ensure coordinated and effective care delivery.
6. Provide ongoing support and counseling to clients and their families, addressing emotional, social, and financial challenges associated with medical conditions.
7. Identify and connect clients with appropriate community resources, support groups, and educational programs to enhance self-management and overall well-being.
8. Monitor and evaluate clients' progress and adjust care plans accordingly, ensuring regular documentation and reporting of outcomes.
9. Maintain accurate and up-to-date client records, ensuring compliance with confidentiality and privacy regulations.
10. Stay abreast of medical advancements, treatment options, and community resources to enhance knowledge and provide informed recommendations to clients.
Qualifications:
1. Bachelor's degree in Social Work, Human Services, or a related field. Master's degree preferred.
2. Valid licensure or certification as a Social Worker or equivalent in the relevant jurisdiction.
3. Proven experience in medical case management, preferably within a healthcare or social services setting.
4. In-depth knowledge of medical conditions, treatment options, and healthcare systems.
5. Familiarity with relevant federal, state, and local regulations, including HIPAA and confidentiality requirements.
6. Strong assessment and care planning skills, with the ability to develop individualized, comprehensive care plans.
7. Excellent communication and interpersonal skills, with the ability to build rapport and establish effective relationships with clients, healthcare professionals, and community resources.
8. Exceptional organizational and time management abilities, with a keen attention to detail.
